The sham-controlled, single-blinded crossover study tested menthol inhalation in 10 healthy adults and measured objective airway resistance, respiratory frequency, and minute ventilation. The key finding is that menthol inhalation did not alter measured upper airway resistance compared to sham — meaning menthol does not act as a true bronchodilator or nasal decongestant in objective terms. However, menthol is a well-established agonist of the cold-sensing TRPM8 receptor in nasal and airway mucosa, which creates a strong subjective sensation of increased airflow and easier breathing without measurably changing actual airway caliber. This "perception vs. physiology" distinction is the central scientific fact about menthol: it powerfully improves the sensation of breathing, particularly relevant for subjective relief of nasal congestion, dyspnea, and breathing discomfort.
