Cordyceps sinensis Improves Exercise Capacity in Older Adults: Pilot RCT

This randomized, double-blind, placebo-controlled pilot study (n=20 adults aged 50–75) administered a standardized Cordyceps sinensis extract (CordyMax Cs-4) at 333 mg three times daily (1 g/day total) for 12 weeks. Compared to placebo, the Cordyceps group showed a trend toward improved metabolic threshold and ventilatory threshold — markers of aerobic exercise capacity — though results did not reach statistical significance in this underpowered pilot. No adverse events were reported. Earlier RCT work (PMID 15903375) in younger adults also showed non-significant trends toward improved VO₂max. Cordyceps' primary mechanistic claim is via adenosine and cordycepin content, which enhance ATP production and oxygen utilization. Preclinical and TCM data strongly support its use for energy metabolism and fatigue resistance, though human RCT evidence at low doses remains limited.

Cochrane Systematic Review: Reishi for Cancer Treatment (5 RCTs, n=373)

This Cochrane review — the highest evidence tier in clinical medicine — pooled data from 5 RCTs examining Reishi in cancer patients. Key findings: patients receiving Reishi alongside conventional chemo/radiotherapy were 1.27 times more likely to respond positively than those receiving conventional treatment alone. Reishi significantly increased CD3, CD4, and CD8 lymphocyte percentages by approximately 3–4% each, with marginal increases in NK cell activity and quality of life scores. Reishi was not found effective as a standalone cancer treatment. The review notes the methodological quality of included studies was generally low and all participants were from Chinese populations — limiting applicability. Minor side effects (nausea, insomnia) were noted; no major hepatological or hematological toxicity was observed. Relevance to this supplement: The doses used in RCTs (typically 1.5–3 g/day of Reishi extract) substantially exceed the ~25 mg equivalent present in this 150 mg blend of five mushrooms. The immune-supportive signal is real but dose-dependency questions are significant.
