Biomarkers of cobalamin (vitamin B12) deficiency and its application

A comprehensive clinical review on B12 biomarkers and deficiency retrieved from PubMed confirms that vitamin B12 (cobalamin) is essential for neurological function through two key pathways: it is a cofactor for methionine synthase (required for myelin synthesis and neurotransmitter methylation, including dopamine) and for methylmalonyl-CoA mutase (required for proper nerve lipid composition). Deficiency — which affects 0.6–46% of the elderly depending on population — leads to irreversible neurological damage including subacute combined degeneration of the spinal cord and cognitive impairment. A doubling of holotranscobalamin (the active B12 fraction) from 50 to 100 pmol/L is associated with a 30% reduction in cognitive decline rate. Crucially, 500 mcg of oral B12 is a very generous and clinically effective dose. The recommended daily allowance is only 2.4 mcg — but because absorption of oral B12 at high doses occurs via passive diffusion (bypassing the intrinsic factor system), approximately 1% of 500 mcg (~5 mcg) is absorbed even in people with absorption disorders. Cordyceps sinensis Improves Exercise Capacity in Older Adults: Pilot RCT

This randomized, double-blind, placebo-controlled pilot study (n=20 adults aged 50–75) administered a standardized Cordyceps sinensis extract (CordyMax Cs-4) at 333 mg three times daily (1 g/day total) for 12 weeks. Compared to placebo, the Cordyceps group showed a trend toward improved metabolic threshold and ventilatory threshold — markers of aerobic exercise capacity — though results did not reach statistical significance in this underpowered pilot. No adverse events were reported. Earlier RCT work (PMID 15903375) in younger adults also showed non-significant trends toward improved VO₂max. Cordyceps' primary mechanistic claim is via adenosine and cordycepin content, which enhance ATP production and oxygen utilization. Preclinical and TCM data strongly support its use for energy metabolism and fatigue resistance, though human RCT evidence at low doses remains limited.

Landmark RCT: Lion's Mane Improves Cognitive Function in Older Adults with Mild Cognitive Impairment

This is the foundational human RCT for Lion's Mane cognition. Fifty-one Japanese adults aged 50–80 with mild cognitive impairment were randomized to 750 mg/day of Hericium erinaceus powder (equivalent to the fruiting body) or placebo for 16 weeks. The Lion's Mane group showed significantly higher scores on the Hasegawa Dementia Scale at weeks 8, 12, and 16 compared to placebo (p<0.05 at all time points). Scores declined after supplementation ceased, confirming the effect was treatment-dependent. No adverse effects were reported. Dose gap note: This study used 750 mg — 7.5 times higher than this supplement's 100 mg extract.
