Iron Supplementation for Unexplained Fatigue in Non-Anaemic Women: Double-Blind Randomised Placebo-Controlled Trial

This landmark RCT in 144 women aged 18–55 with unexplained fatigue — but not clinical anaemia — tested 80 mg/day of elemental iron versus placebo for four weeks. Fatigue scores fell by 29% in the iron group versus only 13% in the placebo group, a statistically significant difference (p=0.004). Crucially, the benefit was restricted to women with serum ferritin concentrations of ≤50 μg/L, indicating that even sub-clinical iron insufficiency — well below the threshold for diagnosed anaemia — is sufficient to cause measurable fatigue that responds to supplementation. This is particularly relevant since ferric saccharate delivers iron in a complexed form known for gentler GI tolerability than ferrous sulphate.

Daily Iron Supplementation for Improving Anaemia, Iron Status and Health in Menstruating Women: Cochrane Systematic Review

This high-quality Cochrane review of 67 RCTs covering 8,506 menstruating women represents the gold-standard evidence base for iron supplementation. High-quality evidence showed that iron significantly raised haemoglobin concentration (mean difference +5.30 g/L, 95% CI 4.14–6.45) and substantially reduced the risk of anaemia (risk ratio 0.39) and iron deficiency (risk ratio 0.62). Iron supplementation also improved maximal and submaximal exercise performance and reduced symptomatic fatigue. Gastrointestinal side effects (constipation, loose stools) were noted as the main trade-off — an important consideration since ferric saccharate, used in this supplement, is typically associated with lower GI intolerance than ferrous forms.

Anemia of Inflammation

This authoritative review in the leading haematology journal explains the bidirectional link between iron, immunity, and red blood cell production. Inflammatory cytokines and the hormone hepcidin restrict iron availability during infection — a deliberate immune response that starves pathogens of iron — but this also suppresses erythropoiesis and can tip into anemia of chronic disease. The review establishes that iron is not merely a passive component of haemoglobin but an active regulator of immune cell function and erythroid differentiation, with iron-restricted erythropoiesis representing a major mechanism of disease-associated anaemia. This provides the physiological foundation for understanding why iron deficiency impairs both red blood cell production and immune defense simultaneously.

The Role of Vitamins and Minerals in Hair Loss: A Review

This comprehensive PubMed and Google Scholar review of 125 human studies covers the evidence for micronutrients in non-scarring alopecia, dedicating substantial attention to iron. Iron deficiency is documented as the most common nutritional deficiency worldwide and is directly linked to telogen effluvium (TE) — diffuse shedding triggered by disruption of the hair cycle. Serum ferritin below 40 ng/dL is widely cited as the threshold below which iron-induced hair loss occurs, and the review finds that most authors recommend iron supplementation in women with hair loss and low/borderline ferritin levels. The same review notes that folate deficiency can cause hair, skin, and nail changes, and that lower red blood cell folate concentrations have been observed in patients with alopecia areata versus healthy controls.
